How do I get a Veteran’s ID card for Cook County
The Military and Veterans Discount Program is jointly administered by the Cook County Clerk's Office and the Cook County Department of Veterans Affairs. Hundreds of businesses in Cook County participate in the program to offer active-duty members of the military and veterans discounts for purchases and retail services. A County-issued identification card is issued to those who sign up for the program.

Who is Eligible and What Documents are Required?
- All veterans and active-duty military with proof of service
- Required documents include:
- Color photo headshot (only if being requested online).
- Proof of Service such as a copy of DD-214, VA Medical Card, Military ID or Retired Military ID.

- IN PERSON: Visit the Cook County Clerk’s Veterans Services Office at 118 N. Clark, Chicago, Room 120. Office hours: 9 a.m. to 5 p.m. Monday through Friday.

When all required documents have been received, the Recorder of Deeds Office will process the information and send by mail, the Cook County Military and Veterans Discount Card, entitling the veteran to discounts with 200 merchants in Cook County.
